Brooke Baker

Brooke Baker RN, Co-Owner, Certified Medicaid Planner

Brooke graduated from Boise State University in 2004 as a Registered Nurse with a degree in Health Science. With over 20 years of experience in community-based care, Brooke takes a straightforward approach based on clinical diagnoses and proven outcomes and dispels common myths about health care and Medicaid.

Jessica Young, MSW, Co-Owner, Certified Medicaid Planner

Jessica fell in love with senior case management when she started working in hospice at just 18 years old. Jessica earned her Bachelor’s degree in Social Sciences from Boise State University and a Master's Degree in Social work with an emphasis in Geriatrics and community mental health from Northwest Nazarene University. She is passionate about helping seniors access the care they need, regardless of their financial status.

Michelle O'Hayre, Case Coordinator Supervisor/Marketing

Michelle has over 20 years experience in customer service, leadership, and office management in the medical and dental professions. Her extensive industry knowledge and personalized approach to each Client case helps set Legacy Navigation apart in the healthcare field.

Melissa Hardy, SUDA, Certified Medicaid Planner

Melissa has completed her Substance Use Disorder degree and is currently working to complete her Bachelor’s in Social Work degree through Grand Canyon University. She has worked in the medical field for over twenty years and combines medical and mental health knowledge with empathy and compassion supported by evidence-based practices. She uses this knowledge to help our clients navigate the myths in the healthcare and Medicaid world.

Taysha Powell, Case Manager

Taysha joined Legacy Navigation in early 2025, bringing more than a decade of experience in the medical field. With a background in medical billing and previous certification as a Nursing Assistant, she offers a well-rounded understanding of both the clinical and financial sides of healthcare.

Her passion lies in helping seniors and their families navigate the financial complexities of qualifying for long-term care benefits. Taysha takes pride in guiding families through each step of the process with clarity, compassion, and attention to detail, ensuring they receive the benefits they need and deserve.

We are an independent certified Medicaid planning agency and not connected with Medicaid. We specifically focus on the 65 and older and disabled populations, and related A&D Medicaid waivers. We are unable to answer questions outside of these programs. For questions regarding other Medicaid programs, SNAP, health plans, etc. please call your local state Medicaid agency.
